These Terms and Conditions (“Terms”) govern the use of services provided by Journeywell Transportation Services LLC (“Journeywell,” “we,” “our,” or “us”). By scheduling or using our non-emergency medical transportation services, you agree to comply with and be bound by these Terms.
Services Provided
Journeywell Transportation Service LLC provides Non-Emergency Medical Transportation (NEMT) services to individuals requiring transportation to and from medical appointments, healthcare facilities, pharmacies, and approved destinations. We do not provide emergency medical services.
If you require emergency medical assistance, please call 911 immediately.
Eligibility and Booking
Transportation services must be scheduled in advance.
Clients must provide accurate and complete information at the time of booking.
Services are subject to availability and service area limitations.
Journeywell reserves the right to refuse service if required information is incomplete or inaccurate.
Client Responsibilities
Clients agree to:
Be ready at the scheduled pickup time
Provide accurate pickup and drop-off locations
Inform Journeywell of any mobility assistance needs in advance
Treat drivers and staff with respect
Failure to comply may result in delays, cancellation of service, or suspension of future bookings.
Wait Time Policy
Drivers may wait up to 10–15 minutes at pickup locations. If a client is not present within the designated wait time, the trip may be considered a “no-show” and subject to cancellation.
Cancellations and No-Shows
Cancellations must be made at least 24 hours in advance, when possible.
Repeated no-shows or late cancellations may result in service restrictions.
Insurance-covered trips may be reported to the payer in accordance with contractual requirements.
Safety and Conduct
For the safety of all passengers:
Seat belts must be worn at all times
Smoking, alcohol, and illegal substances are strictly prohibited
Abusive, threatening, or unsafe behavior will not be tolerated
Journeywell reserves the right to terminate a trip if safety is compromised.
Personal Belongings
Clients are responsible for personal belongings. Journeywell Transportation Service LLC is not liable for lost, damaged, or stolen items left in vehicles.
Service Animals
Service animals are permitted in accordance with applicable laws. Emotional support animals or pets must be disclosed and approved prior to transport.
Payment and Insurance
Some services may be covered by insurance, brokers, or healthcare providers.
Clients are responsible for any charges not covered by insurance.
Payment terms vary based on service agreements.
Journeywell does not guarantee insurance coverage or payment approval.
Limitation of Liability
Journeywell Transportation Service LLC is not responsible for delays caused by traffic, weather conditions, mechanical issues, or circumstances beyond our control. Liability is limited to the extent permitted by law.
Privacy
Client information is handled in accordance with our Privacy Policy. By using our services, you consent to the collection and use of information as described therein.
Changes to Terms
Journeywell reserves the right to modify these Terms and Conditions at any time. Updated terms will be posted on our website with a revised effective date.
Governing Law
These Terms shall be governed and interpreted in accordance with the laws of the State of Illinois.
